The short answer
Spain's public healthcare system (SNS) is tax-funded and widely regarded as one of Europe's best. Access depends on your status: workers and residents are covered automatically, while others can buy in through the "convenio especial" or go private. This article covers the system and the paperwork — not medical advice.
The SNS: how it works
The Sistema Nacional de Salud (SNS) is decentralized: each autonomous community (Andalucía, Cataluña, Madrid…) runs its own health service, but coverage is national — your card works anywhere in Spain.
Primary care starts at your local centro de salud, where you register with a family doctor (médico de cabecera). Specialist visits require a referral from them.
Emergencies go to hospital urgencias — free at the point of use for those covered.
Prescriptions: covered patients pay a copayment on outpatient medicines based on income (the standard range runs from about 10% to 60% depending on income level); pensioners pay reduced rates. Hospital-administered medication is free.
Dental care for adults is mostly not covered (only extractions and emergencies) — a common surprise for newcomers.
Who is covered automatically
You are covered if you are:
Employed or self-employed (autónomo) in Spain — you and your dependents are covered through Social Security contributions.
A pensioner receiving a Spanish pension, or an EU citizen with an S1 form from your home country.
A legal resident without other coverage in some communities (rules vary — Catalonia and others have extended coverage to registered residents regardless of status; check your community's health service).
EU visitors use the European Health Insurance Card (EHIC) for necessary care during short stays — it is not a substitute for residence coverage.
The tarjeta sanitaria individual (TSI): how to get it
The tarjeta sanitaria individual is your personal health card — you need it to see a doctor or pick up prescriptions.
Empadronamiento — register your address at the town hall first.
Social Security affiliation — if you work, your employer does this; if not, go to a TGSS office to get your Social Security number.
Apply at your centro de salud — bring your empadronamiento certificate, ID/NIE, and Social Security document. You choose your family doctor there.
The card arrives by mail in a few weeks; a provisional paper slip covers you meanwhile.
Each community has its own card design and application desk — search "[your community] tarjeta sanitaria individual" for the exact office.
The convenio especial: buying into the public system
If you are a resident with no other route to public coverage (not working, no S1, no bilateral agreement with your country), you can sign the convenio especial de prestación de asistencia sanitaria with your community's health service.
€60/month if you are under 65; €157/month if you are 65 or older (set by Real Decreto 576/2013; communities may add a surcharge for extra regional services).
Requirements: one continuous year of effective residence in Spain immediately before applying, empadronamiento in the community where you apply, and no other access to public healthcare.
It covers the basic common benefits package; outpatient medicines and some other items may be charged at full price.
Private insurance
Private health insurance (seguro médico privado) is common as a complement — shorter specialist waits, private hospitals, English-speaking doctors. Major providers include Sanitas, Adeslas, DKV, and AXA. Basic plans with copayments start around €25–€40/month; full-coverage plans without copayments typically run €50–€80+/month, depending on age and coverage; watch for waiting periods (carencias) on certain treatments and check whether your policy is accepted for visa applications if you need one (some visas require private insurance with no copayments).
Frequently asked questions
I'm an EU citizen — do I need anything special?
For short stays, bring your EHIC. Once resident and working, you join the SNS like a Spaniard. Non-working EU residents typically use the S1 form from their home country or the convenio especial.
Can I choose my doctor?
Yes — you register with a specific médico de cabecera at your centro de salud and can request a change. Choice of specialist is more limited than in the private system.
Are prescriptions free?
No — covered patients pay an income-based copayment at the pharmacy (roughly 10%–60%). Keep receipts if you take regular medication; there are monthly caps for some groups.
What if I need care before my card arrives?
Go to urgencias for emergencies — they cannot turn you away. For non-urgent care, the provisional slip from your centro de salud works.
Does the convenio especial cover dental?
No more than the standard SNS dental coverage (essentially extractions and emergencies for adults). The convenio mirrors the basic public benefits package.
