Travel insurance in Spain: who needs it and how to choose

⚠️ Disclaimer: For general information only — not tax, financial, legal, or medical advice. Figures may be outdated or incorrect; tax rules change frequently. The short answer

Spain's public system (Sistema Nacional de Salud) covers legal, habitual residents — but the paperwork chain from empadronamiento to the health card takes weeks to months, and some visa routes require private insurance by law. This guide covers who needs travel insurance, the TSI process, providers worth comparing, and the Schengen €30,000 rule.

Who needs travel insurance

  • Tourists and Schengen visa applicants: insurance is part of the visa file.
  • Non-lucrative visa (NLV) holders: must hold private health insurance from an insurer authorized in Spain — travel insurance is explicitly not accepted.
  • International students: long-stay student visas generally require private cover for the stay.
  • New residents: bridge the weeks or months before the TSI (Tarjeta Sanitaria Individual) arrives.
  • Visiting parents and relatives: age-rated premiums, pre-existing conditions must be declared.

The public-healthcare path: from empadronamiento to the TSI

  • Step 1: register your address at the town hall (empadronamiento / padrón) — the proof of habitual residence. It is free, and the padrón alone does not create the right to care.
  • Step 2: get a social security number (NUSS); employees get it via the employer, others apply at the Tesorería (form TA.1).
  • Step 3: the INSS (National Social Security Institute) recognizes your right to care and issues the acreditativo document.
  • Step 4: your local centro de salud registers you, assigns a GP and requests the TSI — the individual health card proving entitlement. Each of Spain's 17 autonomous communities issues its own card.
  • People without legal residence have a separate route under Ley 16/2003 art. 3 ter, with a state-level procedure set by RD 180/2026 giving provisional access while the file is decided.
  • The NLV rule, from Spain's foreign ministry: the policy must come from an insurer authorized to operate in Spain, run for one year, cover everything the public system covers, and have no deductible, no copayments (sin copagos), no waiting periods (sin carencias) and no coverage limit. Travel insurance with medical assistance is expressly rejected. Some consulates accept an S1 registration receipt instead of a policy, but wording varies by consulate — check yours.

Providers worth comparing

Names only, no endorsements: SafetyWing and World Nomads for flexible travel cover; IATI as the Spanish-market name. For NLV and student visas, compare only among insurers authorized in Spain (DGSFP-registered) — a cheap international travel policy will fail the consulate checklist.

How to choose: by profile

  • NLV holders: Spanish-authorized insurer, sin copagos, sin carencias, one-year term, and certificate wording matching your consulate's checklist — wording varies by consulate, so read the exact office handling your application.
  • Students: private cover for the visa duration; check the consulate's list before buying.
  • Tourists (Schengen visa): minimum €30,000, valid in all Schengen states for the entire stay, covering emergency care, hospitalization, repatriation and death.
  • Visiting parents: medical limit plus evacuation; pricing jumps at 65+; declare every condition.
  • Reading a policy: medical cap (límite), evacuation and repatriation (repatriación), deductible (franquicia), and direct billing versus reimbursement.

Claims: the practical side

  • Keep every factura, informe médico and discharge paper.
  • Planned care needs written pre-authorization from the insurer before treatment.
  • Declare pre-existing conditions (preexistencias) at purchase — Spanish visa policies screen them explicitly.
  • Confirm direct settlement versus reimbursement before treatment, so you know the cash needed on hand.

Forum pain points

"Which insurance for the non-lucrative visa?" dominates every Spain expat forum: the trap is buying a standard policy with copayments (€5–15 per visit is normal in Spanish private insurance) and having the consulate reject it — the checklist word is sin copagos, and travel insurance is refused outright. The second classic is "which is the cheapest Schengen visa insurance?": the same 2026 answer as the rest of Europe, roughly €1–5 per day for young healthy travelers, with IATI the local name that recurs for Spain trips.

Frequently asked questions

Can I use travel insurance for a non-lucrative visa?

No. Consulates reject travel insurance for the NLV — you need health insurance from an insurer authorized in Spain, with no copayments, no waiting periods and no coverage limit.

Does the padrón alone give me healthcare?

No. The empadronamiento proves your address; the right to care comes through INSS/social security registration or the art. 3 ter route, and the TSI is issued afterwards.

I am an EU citizen. Do I need insurance?

The EHIC covers medically necessary care during temporary stays. For residence, you register through the standard route (S1 or social security) — tourists' EHIC cover is not a substitute for the visa insurance requirement.

Is €30,000 really the minimum for a Schengen visa?

Yes. The EU Visa Code requires at least €30,000 for emergency medical care, hospitalization and repatriation, valid in every Schengen state for the whole stay. Anything less means refusal.

My parents are visiting. What is the key check?

Age-rated pricing and the pre-existing-condition clause. For older travelers, the medical cap and evacuation sub-limit decide more than the premium does.
