La respuesta corta
If you recently moved to the US, health insurance is probably the most confusing system you will meet. The Affordable Care Act (ACA) marketplace's Open Enrollment for 2027 coverage runs November 1, 2026 to January 15, 2027 in most states. Enroll by December 15, 2026 for coverage starting January 1, 2027. This guide covers who can enroll, what subsidies still exist, and the traps newcomers fall into. (This is insurance enrollment information, not medical advice.)
Who can enroll
Lawfully present immigrants — green card holders, work/student visa holders, refugees, asylees, TPS and DACA recipients — can buy marketplace plans. Undocumented immigrants cannot buy marketplace coverage at any price. There is no waiting period for the marketplace: eligibility starts as soon as you are lawfully present, and becoming lawfully present is itself a qualifying life event giving a 60-day Special Enrollment Period. (The 5-year waiting bar applies only to Medicaid/CHIP, not the marketplace.)
The big 2027 change: subsidies narrowed
The enhanced premium tax credits expired December 31, 2025 and were not renewed — the old 400% of federal poverty level "subsidy cliff" is back: one dollar over the line means zero credit. Worse for newcomers: under the One Big Beautiful Bill Act (2025), from January 1, 2027 only US citizens, green card holders, Cuban/Haitian entrants and COFA migrants qualify for premium tax credits. Refugees, asylees, TPS holders and work/student visa holders can still buy plans, but get no subsidies — budget for the full premium.
For 2027 coverage, credits require household income between 100% and 400% of the poverty level (about $15,960–$63,840 for a single person under 2026 guidelines).
How to enroll
Go to healthcare.gov (or your state's own marketplace — California, New York and others run their own, with slightly different dates).
Create an account and enter your immigration documents.
Estimate your 2027 household income carefully — credits are reconciled at tax time (Form 8962), and underestimating means repaying excess credits.
Compare plans by total cost (premium + deductible + out-of-pocket max), not premium alone.
Enroll by December 15 for January 1 coverage.
Newcomer pitfalls
Income estimation with no US tax history is the #1 trap. Be realistic; the IRS reconciles.
Work-visa holders get no 2027 subsidies under the new law — price plans at full cost.
Moving to the US from abroad triggers a 60-day Special Enrollment Period, so late arrivals are not locked out.
State matters: deadlines and Medicaid expansion differ by state.
Preguntas frecuentes
I just got my green card. Can I enroll now?
Yes — becoming lawfully present triggers a 60-day Special Enrollment Period. You don't need to wait for Open Enrollment.
I'm on a work visa. Do I get subsidies?
For 2027 coverage, no — but you can still buy a marketplace plan at full price.
What if I miss January 15?
You generally cannot enroll until next Open Enrollment unless you have a qualifying life event (job loss, marriage, birth, move).
