Prescriptions in China: Hospital Pharmacies, Drugstores, Online Sales, and Insurance
Getting medicine in China involves more channels than most newcomers expect: hospital pharmacies, retail drugstores, internet hospitals, and online platforms — each with different rules about prescriptions and insurance. Here's how they fit together.
The Standard Flow: Hospital Prescription
After your consultation, the doctor issues an electronic prescription (电子处方). You pay at the billing step, then collect the drugs at the hospital pharmacy (药房/取药处) with your payment record. Controlled substances (麻醉药品、精神药品) have strict quantity limits and ID checks — expect paperwork.
Tip: hospital pharmacies dispense exactly what the doctor ordered. If you want a substitute or have questions about side effects, ask the doctor before you leave the consultation room, or consult the pharmacy window pharmacist (they're licensed and helpful). For chronic medications, ask whether a larger quantity (e.g., a 30-day supply instead of 7 days) can be prescribed at once — many hospitals now allow extended prescriptions (长处方) for stable chronic conditions, saving you repeat visits.
处方外流: Taking Your Prescription Elsewhere
Hospitals increasingly support 处方外流 (prescription outflow): instead of collecting drugs at the hospital, you take the electronic prescription to a designated retail pharmacy — often cheaper or more convenient, especially for chronic medications. Ask the doctor or the hospital app whether your prescription supports外流; not all drugs or hospitals do yet.
Retail Drugstores (药店)
Walk-in pharmacies are everywhere (国大药房、老百姓大药房、海王星辰, plus countless independents). Rules:
- OTC drugs (非处方药): buy freely, including most cold medicine, vitamins, bandages.
- Prescription drugs (处方药): require a valid prescription. Many chain pharmacies have an on-duty pharmacist who can issue a simple prescription after a quick consultation for common conditions — legal and routine.
- 医保定点药店 (designated pharmacies): look for the 医保定点 sign — you can pay with your insurance individual account (personal account funds) or e-code here, for both OTC and prescription drugs.
Antibiotics are technically prescription-only; enforcement varies, but the compliant path is a prescription.
医保目录 and 集采: Why Some Drugs Are Cheap
Drugs on the national insurance drug list (医保药品目录) are reimbursable; the list is updated annually. Separately, the centralized procurement program (药品集中采购, 集采) — running since the 2018 "4+7" pilot — has slashed prices of many common generics by letting provinces bulk-buy. If your prescribed drug seems surprisingly cheap, 集采 is probably why. Ask for the generic (集采中选) version if cost matters.
Internet Hospitals (互联网医院)
For follow-up consultations and refills of chronic medications, internet hospitals (run by major public hospitals or platforms like 微医、好大夫在线、京东健康) let you video/text-consult a licensed doctor who issues an electronic prescription, with drugs shipped to your door. First visits for new or serious conditions still belong offline; online is for stable, known conditions.
Buying Prescription Drugs Online
Since the 《药品网络销售监督管理办法》 took effect on December 1, 2022, prescription drugs can legally be sold online — but only with a valid electronic prescription issued after an online consultation. Platforms (京东大药房、阿里健康大药房, etc.) build the consultation into the checkout flow: you describe your condition, a licensed pharmacist/doctor reviews it, and the prescription is issued on the platform. Vaccines, controlled substances, and certain special drugs are excluded from online sale.
Traditional Chinese Medicine (中药)
TCM has its own parallel track. At hospital TCM departments or dedicated 中医院, doctors prescribe herbal formulas (汤剂) — raw herbs you boil at home — or granules (配方颗粒) that dissolve in hot water. Most hospitals offer 代煎 (decoction service): the pharmacy boils, vacuum-packs, and sometimes delivers the medicine for a small fee — far more practical than doing it yourself.
中成药 (ready-made TCM products like pills and capsules) are widely available OTC at pharmacies. Note the insurance distinction: TCM consultations and in-catalogue herbal formulas are reimbursable; many patent medicines and most private TCM clinics are self-pay. As with Western drugs, don't mix TCM and Western prescriptions without telling both doctors.
Frequently asked questions
Can I use insurance to buy drugs at a pharmacy?
Yes, at 医保定点 pharmacies, using your individual account balance or insurance e-code. Reimbursement rules for pharmacy purchases follow your city's policy.
What if the hospital pharmacy is out of stock?
Ask the doctor for an alternative prescription or a 处方外流 to a retail pharmacy. For rare drugs, the hospital's procurement office can sometimes order it.
Are imported drugs available?
Yes, at hospital pharmacies and large chains, but they're usually off-insurance (self-pay) and expensive. Domestic generics that passed consistency evaluation (一致性评价) are the cost-effective default.
